As a Support Specialist at ECAS, you will help clients remain connected, engaged, and supported in the ways that matter most to them. From accompanying clients to restaurants, museums, shopping trips, the gym, appointments, and community outings to assisting with errands, personal shopping, and everyday needs, you will provide thoughtful, individualized support that enhances quality of life.

What You'll Do

Who We're Looking For

As a Care Manager at ECAS, you will serve as a trusted advisor and advocate for individuals and families navigating complex health, aging, and care needs. Our Care Managers take a thoughtful, highly personalized approach to understanding each client's circumstances, identifying needs, coordinating services, and developing solutions that support safety, independence, dignity, and quality of life.

This role requires exceptional judgment, communication, and follow-through. You will work closely with clients, families, healthcare providers, community professionals, and the broader ECAS team to ensure every aspect of a client's care is thoughtfully coordinated. The ideal Care Manager combines clinical knowledge with compassion, confidence, and the ability to bring clarity and calm to complicated situations.

What You'll Do

Conduct comprehensive assessments to understand each client's health, psychosocial, environmental, and day-to-day needs

Develop and implement individualized care plans based on each client's goals, preferences, abilities, and circumstances

Coordinate medical care, home care, therapies, community services, appointments, and other resources

Serve as a trusted point of communication among clients, families, physicians, healthcare providers, and other professionals

Advocate for clients and help ensure their wishes, needs, dignity, and best interests remain at the center of decision-making

Attend medical appointments, care conferences, hospital or rehabilitation meetings, and other important discussions as needed
-
Monitor changes in a client's health, cognition, function, living environment, or overall well-being and respond appropriately

Help clients and families navigate transitions in care, including hospitalizations, rehabilitation stays, changes in living arrangements, and increased support needs

Provide families with clear, thoughtful guidance during complex or sensitive situations

Maintain timely, thorough, and professional documentation

Collaborate closely with ECAS Care Associates, Support Specialists, guardians, and other members of the care team

Develop strong relationships with healthcare providers, senior living communities, home care agencies, and other trusted community resources

Maintain the highest standards of confidentiality, professional boundaries, ethics, and discretion
Who We're Looking For
-
3+ years of professional experience in care management, nursing, social work, aging services, case management, or a closely related field
-
Bachelor's degree or higher in nursing, social work, gerontology, psychology, counseling, healthcare, or a related discipline preferred
-
Registered Nurse (RN), Licensed Social Worker (LSW/LCSW), or comparable professional background strongly preferred
-
Care Manager Certified (CMC), Certified Case Manager (CCM), Certified Social Work Case Manager (C-SWCM), Certified Advanced Social Work Case Manager (C-ASWCM), or another relevant professional certification preferred
-
Experience working with older adults, individuals with disabilities, individuals with cognitive impairment, or families navigating complex care needs

Excellent written and verbal communication skills

Strong clinical judgment, critical thinking, problem-solving, and decision-making abilities

Exceptional organizational skills and the ability to manage multiple clients, priorities, providers, and deadlines

A collaborative, team-oriented approach and willingness to communicate openly with colleagues

Confidence interacting with physicians, attorneys, healthcare professionals, families, facilities, and other community partners

A compassionate, composed presence, particularly during challenging or emotionally sensitive situations

As a Care Associate at ECAS, you will provide thoughtful, personalized support to clients in the comfort of their homes. This role goes beyond traditional assistance: it is about creating meaningful moments, building genuine connections, and helping each client feel comfortable, engaged, and valued.
Whether spending time together over a favorite activity, working on a craft, organizing a meaningful space, tidying the home, or simply sitting down to listen and learn more about a client's life, our Care Associates bring warmth and intention to everyday moments. We are looking for individuals who naturally notice the little things and understand that exceptional care is often found in the details.

What You'll Do

Provide meaningful companionship and one-on-one engagement in the client's home

Spend time getting to know each client's personality, interests, preferences, routines, and life story

Plan and participate in enjoyable at-home activities such as crafts, games, puzzles, music, conversation, or other interests

Assist with light tidying and maintaining a comfortable, welcoming home environment

Help organize personal belongings, closets, drawers, paperwork, or other household areas as appropriate

Support clients with everyday tasks while encouraging independence whenever possible

Anticipate small needs and identify thoughtful ways to make a client's day more comfortable or enjoyable
